Appendix H to Part 40—DOT Drug and Alcohol Testing Management Information System (MIS) Data Collection Form

The Department of Transportation (DOT) has issued Appendix H to Part 40, which is the management information system (MIS) data collection form for drug and alcohol testing. This form is required for reporting calendar year MIS data and the instructions can be found at the DOT’s Office of Drug and Alcohol Policy and Compliance website. The form must include information such as the total number of tests conducted, the number of positive, negative, and cancelled or invalid test results, and information on other testing-related activities. The form also includes a section for employers to report their drug and alcohol program violations. The purpose of the MIS data collection form is to help the DOT monitor and evaluate the effectiveness of drug and alcohol testing programs and to identify areas where improvements may be needed.
