Create New Test Request
New Test Request Overview
To initiate a new employee test request in Med-Stop, please follow the steps below:
1. Define Test Parameters
Select the Service Category, specify the reason for the test, and indicate the DOT status.
2. Provide Special Instructions
Notify the collection site of any additional instructions relevant to the test request.
3. Set Test Date and Time
Specify the date of the test and the latest time the test should be completed.
4. Select a Test Location
You can choose the location using any of the following methods:
By ZIP Code:
Enter the desired postal ZIP code and click "Find Location". A list of available locations within the specified range will appear. Select your preferred site and review its details.
From Preferred Locations:
Your company can maintain a list of preferred testing sites. Only locations that meet the test type and date criteria will be shown.
From Map View:
Click the "Map" button to visually locate and select a testing site.
Manual Entry:
If no matching location is found, you may enter the address manually. However, always try to find the location using the tools above before entering it manually.
5. Finalize and Submit
After selecting the location, review the details and proceed to complete the test request.
If you wish to print the test request notification:
Set the "Show Additional Tasks" option to YES.
Then select "View / Print Test Request Notification" on the final screen.
