Below are the forms that you can use for your employment process:
Application for Employment
This is a standard application form for employment. You have the option to select an existing applicant or print empty forms.
Employment History Verification
This form is used to verify the safety history of an employee from their previous employers.
Request for Information from Previous Employer
This form is used to record contacts with previous employers.
Request for Check of Driving Record
This form helps satisfy the driver qualification requirements by checking the driving record of an applicant.
Request for Drug and Alcohol Records
This form is used to request drug and alcohol testing records of an applicant from their previous employer. You have the option to select an existing applicant or print an empty form.
Employee Eligibility Verification
The Employee Eligibility Verification (I-9) is the form required by the Department of Homeland Security - U.S. Citizenship and Immigration Services to document eligibility for employment in the United States.
Fair Credit Reporting Act Disclosure
This form helps you satisfy the requirements of Section 604(b)(2) of the Fair Credit Reporting Act. It states that a consumer reporting agency cannot give out your information to your employer or potential employer without your written consent. Written consent is generally not required in the trucking industry.
Driver Statement of On-Duty Hours
Commercial vehicle drivers have limitations on the number of hours they can drive. This form is used to record their on-duty hours.
Checklist for Qualification of New Drivers
This checklist helps satisfy the driver qualification requirements of 49 CFR Section 391.51.
Previous Employee Safety Performance History
Prospective employers are required to obtain accident and drug and alcohol testing history from all DOT-regulated employers that the driver-applicant worked for in the previous three years. The request must include a release form signed by the driver-applicant regarding their drug and alcohol testing information. Employers must provide this information to the requesting company within 30 days.
Certificate of Compliance
This is an employee certificate of compliance.
Certificate of Driver Training
This is a certificate of driver training.
Certificate of Road Test
If the road test is successfully completed, the person who administered it should complete a certificate of the driver's road test. The original or a copy of the certificate should be kept in the employing motor carrier's driver qualification file of the person examined, and a copy should be given to the person who was examined.
Certificate of Violations/Annual Review
At least once every 12 months, a motor carrier must review the driving record of each driver. A note stating the results of this review should be included in the driver's qualification file.
